in Pregnant Women
The complications and treats The accurate and timely diagnosis of appendicitis during pregnancy is difficult and the potential for complications is higher. The presentation may be confused with more common obstetric or gynecologic complications. The presence of two patients, fetus and mother, and their competing needs, further complicates the diagnosis and management of appendicitis during pregnancy. Diagnostic accuracy during pregnancy is limited. The visceral component of the pain is perceived in the epigastric or periumbilical area and remains unchanged by pregnancy; however, somatic pain changes with the site of the appendix. In early pregnancy, the location of pain is similar to that observed in the non-pregnant state. As the uterus enlarges and becomes an abdominal organ during the second and third trimester, the position of the appendix becomes higher. Concern for possible fetal infection and pre-term delivery may lead to a delay in surgical therapy with important maternal consequences. Similarly, concern for fetal well being both pre- and postoperatively, complicates maternal convalescence. Maternal complications are more common (and severe) after perforation cases. However, complications may occur even in the absence of perforation. In general, complications are more common as pregnancy advances.
